Clinics for children are held for temporary medical
care, school physicals and sports physicals.  
Children are seen Monday -Friday 9 - 5 p.m.   
Appointments are preferred.  All children must
have a parent present and bring proof of family
income.
Clinics for adults are held for patients with high
blood pressure or diabetes and provide full medical
care, including medications, diagnostic and
preventative screenings and visits with a medical
provider.  
  • Adult services are by appointment only
    Monday - Friday.   
  • Physical Therapy appoints are offered
    Wednesday 2-5.
  • Diabetes Education is provided individually
    and through group classes and are scheduled
    at the patient's appointment.
  • Mental health counseling is provided
    individually and in groups.

OUR MISSION:  to provide health care and access to health care to low income residents in our
community through a network of dedicated volunteers
Patients are eligible for clinic
  • Have a household income of 200%
    or less of the federal poverty level.
  • Have no insurance or
    medicaid/medicare.

The clinic is open Monday - Wednesday
9:00 a.m. to 5:00 p.m.; Thursday
9:00 a.m. to 9:00 p.m. and Friday,
9:00 a.m. to 4:00 p.m.
