blood pressure or diabetes and provide full medical
care, including medications, diagnostic and
preventative screenings and visits with a medical
provider.

Monday - Friday. - Physical Therapy appoints are offered
Wednesday 2-5. - Diabetes Education is provided individually
and through group classes and are scheduled
at the patient's appointment. - Mental health counseling is provided
individually and in groups.

The clinic is open Monday - Wednesday 9:00 a.m. to 5:00 p.m.; Thursday 9:00 a.m. to 9:00 p.m. and Friday, 9:00 a.m. to 4:00 p.m. |
